Commonwealth Festival of Lifesaving – Championship Entry

This form is for entering the Commonwealth Championships 2019 – for National and Development Teams.

On competition of this form the team manager will be sent log details to enter the athlete details and entry times. An invoice, with payment details will follow.

For international visitors who wish to pay for the both the championships, the challenge and closing dinner on one invoice please enter the number of places required.
