GDPR NOTICE: To promote your Restart a Heart activities, we need to collect and process personal data in accordance with the UK GDPR and the DPA18. We request your organisation’s name and any additional information about your activities, as this is necessary for us to promote your Restart a Heart activities. We collect your email address and ATC/P society number to identify and eliminate duplicates. Occasionally, we may contact you to ask for images of your activities so we can share these on our media channels, alongside the information you provide, additionally we may contact you to request your participation for future Restart a Heart communications. We process this information under UK GDPR Article 6(1)(f), as we have legitimate interests in promoting your activities. We will not use the collected data for any other purpose and only collect the necessary information to carry this out. By completing this form, you agree to allow us to promote your business’s Restart a Heart activities. Information provided will be retained for the duration of Restart a Heart month plus an additional 6 months to facilitate the creation and management of any promotions.
